The Prevention Assessment Team (PAT) will hold a monthly drop-in at Horsham Library on the first Wednesday of each month, 10am–1pm, offering advice and support to help residents live independently and stay connected with their community.
The multi-disciplinary team includes Community Prevention Officers, NHS Nurse Advisors and Age UK WSBH Community Support Workers. Blood pressure checks are also available (except 1 September), along with bookable one-to-one appointments.
